Senior Clinical Pharmacist

Job Title: Senior Clinical Pharmacist in General Practice – Icknield Primary Care
Network (PCN)
Location: Letchworth and Baldock, North Hertfordshire
Hours: Full-time – nominally 37.5 per week
Salary: £42,840 – £49,980 pa dependent on qualifications and experience
Contract Type: Fixed Term Contract ending 31 March 2024 (with potential to extend
beyond)

About The Role

Would you like the opportunity to play an active leadership role in supporting the effective delivery of patient care in GP Practices within a defined North Hertfordshire Primary Care Network (PCN) by utilising your pharmaceutical and managerial skills to embed the principles of medicines optimisation within the wider practice teams?

If so, you could be the qualified and experienced Senior Clinical Pharmacist that we are hoping to welcome into our North Hertfordshire PCN and work alongside our PCN GP Practices’ multi-disciplinary teams.

This role is pivotal to improving the quality of care and operational efficiencies so requires a pro-active approach, good motivation and passion to deliver an excellent service within a General Practice environment.

The key priorities for this role are to:

  • Provide supervision and mentorship to the appointed clinical pharmacists working across the PCN GP Practices.
  • Ensure optimum benefit for prescribed medication through high levels of compliance and concordance achieved by support for the patient and carers
  • Provide professional input to PCN Practice teams and network multidisciplinary teams working to improve patient outcomes and reduce avoidable admission to hospital
  • Liaise across providers to ensure accurate and effective medication management
  • Provide direct care to patients with long term conditions, minor ailments and other clinical areas within the scope of practice
  • Support the care of complex patients in their own homes or in nursing or residential accommodation as part of a multidisciplinary team
  • Provide professional leadership on matters relating to medicines optimisation and quality improvement
  • Lead on the prescribing domains of the Quality & Outcome Framework (QoF)
  • Lead on the prescribing domains of the Consolidated Funding Framework (CFF)
  • Monitor and manage the effectiveness of the Clinical Pharmacists’ activities in the Practices

This post is part of the Additional Roles Reimbursement Scheme for PCNs funded by NHS England and includes funded access to the Clinical Pharmacist Training Pathway for any further modules required to complete the pathway.  The contract is therefore being offered as fixed-term to the end of March 2024 with the potential, and expectation, to extend beyond.

About The Candidate

The ideal candidate will have the following skills:

  • Completion of Pharmacy Degree and have at least 5 years post graduate experience.
  • Ideally will have Independent Prescriber status.
  • Excellent clinical and medication knowledge.
  • Proven managerial experience.
  • Understanding and experience of working as a pharmacist in a GP Practice setting, with a confident approach.
  • Experience of undertaking medication reviews.
  • The commitment to continue to learn and develop.
  • The ability to be adaptable and flexible and be a proactive problem solver.
  • Excellent inter-personal patient facing communication skills.
  • Excellent IT skills ‐ ability to use Word, Excel, PowerPoint and Internet to obtain and analyse information.
  • Registered with the General Pharmaceutical Council.
  • Will hold Personal Professional Indemnity Insurance.

About Us

12PointCare is the GP Federation whose members are the ten GP Practices in North Hertfordshire with an overall patient population of circa 100,000.   We support the two Primary Care Networks (PCNs) in North Hertfordshire: Icknield PCN and Hitchin & Whitwell PCN.

The post-holder will work with Icknield PCN which comprises the six GP Practices located in Letchworth, Baldock and Ashwell with a combined patient population of circa 55,000.

All of our Practices operate the SystmOne clinical system, are well-respected, friendly and are well-rated by the CQC.

We have part-time and full-time roles available.

  • Provide professional input to PCN Practice teams and network multidisciplinary teams working to improve patient outcomes and reduce avoidable admission to hospital.
  • Ensure optimum benefit for prescribed medication through high levels of compliance and concordance achieved by support for the patient and carers.
  • Reconcile medicines following hospital discharge, amend medication lists and contact patients/ carers regarding such changes, as appropriate, to ensure patients receive the medicines they need post-discharge.
  • Liaise across providers to ensure accurate and effective medication management.
  • Work as part of a multi‐disciplinary team in a patient facing role, based either remotely or within a member GP Practice, but working for all GP Practices across the PCN.
  • Provide direct care to patients with long term conditions, minor ailments and other clinical areas within the scope of practice.
  • Review MHRA alerts, organise appropriate searches/audits, implement drug withdrawals, etc; improving medicines safety.
  • Support the care of complex patients in their own homes or in nursing or residential accommodation as part of a multidisciplinary team.
  • Provide professional leadership on matters relating to medicines optimisation and quality improvement.
  • Lead on the prescribing domains of the Quality & Outcome Framework (QoF).
  • Lead on the prescribing domains of the Consolidated Funding Framework (CFF).
  • Act as key contact in Practice for all medication related queries.
  • To support improvements in clinical care through Practice-based audits and implementing change.
